Simply put, remote patient monitoring refers to the use of a specific technology to facilitate interaction between clinicians and patients at home. You could be mailed a blood pressure cuff, scale, glucose machine or oxygen checker depending on your needs. The data from the devices are uploaded and communicated to your provider’s care team. Your insurance will be checked for coverage by the supply company, and an order will be placed to a supply company who will reach out to you once you have received the equipment to help you get it set up.

The patient will be mailed a possible device (blood pressure, scale, glucose machine or spo2), and the supply company and care staff will help you get it set up. The devices communicate securely with your provider’s care team. You often will be responsible for taking your blood pressure, glucose or weight once or twice daily as instructed by your provider. Often the provider may reach out to you to adjust your blood pressure medications or insulin depending on your tabulated results.

This depends on your health conditions, your insurance, and if your current condition requires intensive monitoring.
